We are happy to cooperate with you on insurance coverage by completing the "Dentist's Portion of all forms pertaining to your claim. We ask that you carefully read your policy and other available information so you are fully aware of any limitations and procedures. All patients are financially responsible for their own accounts. Our policy is for parents to pay us that portion of the fee not covered by the insurance company. Insurance payments, if possible, should come to our office. Therefore, the portion of the application that assigns the insurance payments to the dentist should be signed. As a general rule, we do not send the original x-rays, casts or any other diagnostic materials to the insurance companies.

Insurance forms
You should obtain the necessary forms with instructions for us. We will do our best to file these in an efficient manner and send them to the insurance company. You should see to it that our office has enough forms. Some companies request billing forms every 3 months, others every year, etc. It is sometimes necessary for us to receive an authorization from the company stating you will be covered. If a person is covered under more than 1 insurance plan, by law we must submit to all companies. The primary carrier is always determined by the birth date of the employee. The person whose birth date occurs first during the year is the primary carrier.
